|Ontario Curriculum by Grade or Subject
The layout of student assessment booklets for 2015 has changed. Students will now record all of their responses—both multiple-choice and open-response—in a separate Answer Booklet. This change is necessary to support online scoring, which will start for all assessments in 2015 as part of the “EQAO Online” project to move the provincial assessment program online.
Examples of the new assessment booklet layout for 2014–2015 are available below.

Additional Tasks: These tasks have been used in a previous assessment. Teachers may use them as instructional tools or for classroom assessment with students in either applied or academic programs.

This guide describes the two EQAO tests Ontario students write in secondary school (available for download in 21 languages).
Mathematical formulas are provided on the Formula Sheets for student reference during the assessment. Formulas are given for perimeter, area and volume for various geometric figures; the academic formula sheet also includes surface area.
